Individual Partnership, Corporation, Limited Liability Company and Unincorporated Business
Everything from Individual column plus the following:

  • Your employer name, address and phone number.
  • Bank or third-party broker account numbers for funding purposes, plus routing number for check deposits.

Family Office Clients

For each of its clients, a Family Office should submit:


If the client is an individual,
the same identification documents that are required of the Family Office itself except that the document submitted as proof of the client's identity (i.e., the clients passport, driver's license, national identity card or alien identification card) must include the client's signature.


If the client is an organization,
the same identification documents that are required of the Family Office itself with the following exceptions:

  • You do not need to submit proof of the organization's registration or license.
  • You should submit one of the following for each of the officers of the organization that has the authority to sign for the organization (i.e., its Controlling Officer, Treasurer and/or Secretary:
Proof of identity, date of birth, and signature.
The document that the applicant provides must include the applicant's name, date of birth, and signature.
  • Driver's License;
  • Passport;
  • Other Government-issued Identification Card.

If the client is a trust, you should submit:
